JenCare complaint: Dr. Refuses to sign your insurance form

My complaint:

After a late evening accident stepping on wood. The next morning I went to doctor to be checked and since it’s been many years since I had a tetanus shot I thought I should be seen since I am diabetic and have had severe leg and foot issues over the past several years. The doctor seen me said it was open but did not see any wood in it. She never mentioned a shot. I gave a very short form for her to fill out for Aflac needing the procedure codes and description and her signature and license number. This is a must for Aflac to prove you were there and seen a doctor. She refused to sign it and the office manager talked to her and she still refused. Tomorrow I was told the office manager will talk to the regional medical director about it. I don’t know if I can believe the office manager as twice he PROMISED to call me back but never bothered. This seems to happen to me a lot where people don’t get their messages. I see online that if I want to or shall I say have to I can contact the BBB. I pay over $50 a month for that policy and this is the first accident I went to my doctor for and she still refuses to fill it out. Any suggestions?
