Dignity Health complaint: 10 requests and unable to obtain health records

My complaint:

I have contacted Chandler regional dignity health and Dignity health urgent care on German Rd multiple times and am unable to obtain medical records.

I was advised to log into the portal. I have. There are no records. I was advised to call technical support. I have multiple times. They advised me to call the locations of treatment. I have. I was given billing contacts vs records. I was told to go to the dignity health website and find the records form and email it in. There are over 10k patient forms on the site and none related to my request. There is no email to send the request to. Then I was told to go in person to request the record is because the system changes since 2018-2020 when I received care. I live outside the state. I cannot go in person. I need these records for income and filing for medical accommodation. It should not be this hard.

Suggested solution:

Allow my access to my records.
