My complaint:
I went to the greenwood location to get a referral to continue physical theraphy at the cora physical therapy location adjacent to the afc location as wells as get a prescription for my medications. i was charged and screened by a woman named lauri that was very disrespectful. she questioned me about things that were unrealated to why i was there and i explained to her that i have a lawsuit and should not be asked certain questions outside the presence of my attorney. she stormed out of the room left me there for over 3 hours past closing time. i told her i felt like she was interrogating me. as i was leaving i was denied copies of my medical records and was told that my discharge papers were actually my medcal records.. i went to the pharmacy and discovered that lauri only prescribed me 1 of my 4 medications. i came back several days later and talked with ashley who gave me my medical records which is when i discoveed that lauri lied in my medical records.
Lauri agreed that my records were a bit strange. i asked to file a complaint and was tld to come back in 2 days and the issue would be fixed. i came back in 2 days and was told ashley was on vaction and was charged again disrespected by office. i was told i was being reinbursted because i payed twice and shouldnt have been because i didnt recieve all my medications the first visit and only came back to have my medications prescribed which shouldve been prescribbed the first vist.staff yet again denied making a complaint, denied phone to corporate office. a few days later i came back and ashley put me on the phone with someone that claimed they would fix the issue.
Too date the issue has not been fixed nor have my funds been reinburst to my account. i have 2 lawsuits that going have great reason to blieve that luari has been compromised working with the oppsition. i called and left messages at 205 588-2461 as well as 864 713-1518 and have yet not recieved a response. i do not want to include afc as a defandant in any 1 of these lawsuits. please contact me asap rgarding this matter for i will be left with no choice but to file with the clerk of court. thanks.
Suggested solution:
i need my medical records corrected as soon as possible as damage has already been done because cora physical theraphy was already supplied with the fraudullent documents. iI cora physical therapy forward those documents to insurance companies then thats even more damages. i work from 7am to 4 pm im on lunch from 11:30 to 12:30 and i expect a phone call or email at your earliest conveinence.
