PHH Mortgage complaint: Violation of my chapter 7

I am sole borrower of a mortgage with phh mortgage in which I filled chapter 7 bankruptcy under I believe case **** and my debt was discharged back in 2016. However, a 3rd party unauthorized person filed chapter 13 under case **** on my debt listing my debt as her creditor which is a lie. This person is my cousin which I did quick claim deed in her name when I will ill.
Since then the bank has continued to collect payments under my name and loan without my permission refusing to close my case. The bank continues to send me monthly billing statements to collect on my debt that has already been discharged. The bank continues to purchase home owners insurance under my name. The prior mortgage company reported negative reporting on my credit because the unauthorized 3rd party continues to make late payments under my name. This is a violation of my right to privacy of my personal information and bankruptcy and I need help resolving this matter I feel is against the law.
