My complaint:

We get pizza from here at least once a week. When I called to order pizza today, the worker had absolutely no phone manners seemed clueless about taking my order, I had no idea what he was saying and after I thought he had my order, I hung up after saying goodbye and no response. I ordered a supreme deep dish without mushrooms. The way I order it every time, however only one time it was ever made without mushrooms. He gave me 3 different totals for the price. When we got to the store to pick up the pizza the kind girl came to the window and told us, that someone made the pizza wrong. They made a deep dish pizza with onions, green pepper, mushrooms and extra cheese. No meats. So the girl at the counter said they would have to remake the pizza. My husband parked the car and went in. He said the place was dirty. The guy making the pizza had huge dread knots in his hair, no hairnet. The first thing my husband said when he got to the car, was, after being in that place, I believe I have lost my appetite. This particular pizza shop is always busy. I would think that they have a very good business but they need to make some improvements

Suggested solution:

They should absolutely clean the store! The workers should be appropriately attired for working with food. The worker who took my order needs to be retrained or at least have an attitude that he even cared or was trying. It was obvious he was lacking.
