My complaint:

I’ve been trying to contact customer service for over a week now. I’ve been trying to get a replacement card. I had to send in two forms of ID. I been calling to see if anyone has received the information and I can’t get anyone on the line.

I would like to receive my replacement card to the facility that I am in. Maplewood Healthcare and Rehab here in Jackson TN
