My complaint:
I recently moved from NY 11754 to WI 53132 n Verizon is not serving this area. They said fill out this form and we will waive the fee. They never waved the fee and I am in collection. Also I over paid $120 and have $79 in collection. I have copies of form sent in and charges. No one cares they just keep say send more money Its an obvious mistake but they dont want to look into it. Thanks

By reading the form with the new address and repaying the money after seeing there is no service in the new area I moved. I have copies I submitted of the forms
