My complaint:

They sent my card to the wrong address my current card expired October 2020 now my benefits is on the card that they sent to Yonkers and I’m having a hard time getting a replacement card how fax over documents they claim they never got them

Suggested solution:

Send my card to my right address . 117 West 138th Street Apartment 1B New York New York 10030
