My complaint:
Second occurence of stop payment due to identity verification! 1st occurrence in May 2020! I provided them UIA my drivers Liscence and birth certificate! 2nd occurrence on 0417202! I provided them my drivers license and birth certificate to verify my identity! That didn’t do anything! Then UIA requested that I send them a selfie of myself, holding my drivers license and birth certificate and that to wasn’t sufficient! Now on 3 occasions they said they would send Form 6347 to my home address and never recieve that form! If that form was sent to my home then it would have been filled out and sent back to them asap! They also want me to give my identity documents again! My claim has been under review for 109 days and they are giving me a run around! If someone wanted their unemployment pay wouldn’t they fill that out ASAP! I have contacted my state representative and that didn’t bring any result! I wish someone could see that this wrong please help me?
