Applebee’s complaint: Retaliatory action by employer

My complaint:

Hello I’m a disabled woman who recently was hired by Applebee’s. I was having some trouble with my performance so I just closed my invisible disabilities and requested accommodations. My accommodations were simple shorter shifts and emergency brakes when necessary for my PTSD condition. Since making the request my hours have been cut to 4 hours a week. Additionally I was written up for submitting a doctor’s note after a dental procedure on Monday that was still problematic by my work shift on Wednesday. I drove to Applebee’s and turn the note in first thing Monday morning but was still written up. Additionally I found increased criticism regarding my required uniform I was told I would be written up for wearing black stretch pants like I see I’m just about every other waitress. Recently I requested pay stubs for the department of health and human services and notified them of my deadline. I was told I could not access these pay stubs myself so 3 days ago I put a request in for personnel to send them over. Today is an important deadline and I’ve received no call back from the manager I’ve spoken with and I’m told she doesn’t know what to tell me about the pay stubs but she’ll let me know when they come in. I’d like a message for Liz Allen in personnel.
This delay will greatly affect my quality of life and seems hypocritical considering I was written up for turning a document in in an untimely manner. I feel this is definitely backlash and I’m considering involving an ADA liaison. I’m hoping you can help me resolve these matters so we can prevent these kind of incidents happening in the future with this company. Thank you for your time.

Suggested solution:

I believe they should resolve my complaint scheduling a meeting with personnel and an ADA liaison or representative. I believe they should destroy the write-up because I had no way to know my face would be swelling up days later when I had to work a shift. Finally I would like my hours back, and a formal apology regarding Ada retaliation.
