My complaint:
I request on April 10, 2023 full Medical history of my surgery and process of my case. They stated that had been mail to me on April 14, 2023 and I DID NOT received any CD OR documentation regarding my Medical history at all. I request that because I fill cheated with final process of that case.
My denture was delivered on February 9, 2023 however completely dysfunctional and not fitted properly. Reason for that is that original screws of my case was not installed only not fitted other screws not assigned to my case! So who’s responsible for that? Doctor Mehrali who did a denture or Burbank Lab who was providing?
I also request meeting regarding that case with Lead Professor – Doctor @ Maxilifacial and Oral surgery department but completely has been ignored. If I cannot obtain my request by this complaint I will have no choice to start a law suite against you.
As far my surgery was gone successfully and perfect way finalizing my case was horrible and out of control. My denture doesn’t fit I cannot eat, my bite is not restore , denture is wobbly and do not hold on the right implant at all.
I know screws assign to my case – original one was big and definitely will hold when this one are not attached properly and not compatible to my implants. Finally said I cannot function properly. As a proof I’m sending original screws should be n my case and I don’t have nothing line this.
I request investigation who is responsible for crappy job was done to me. I wanna mention that I had tumor in my lower jaw being completely toothless and I wanna normal functioning especially when also my job request mouth and talk.
Mine doesn’t look like this at all. Is nothing to hold the denture in place and plastic part of this is breaking every 2 weeks.
I already spent extra $550 to be able to hold dysfunctional denture in place on one site.

I wanna see a Head Doctor from Maxillofacial and Oral surgery dental department asap or Hospital Director. I would like to provide a proof of negligent job and cheating on patient care.
Also I request full copy of medical history ( including doctor Schiffler notes and Dr. Jimmy notes of 32 pages. Is important because is information what kind of implant system was assign to me.
