My complaint:
Sept 30 2019. Went in the bathroom to do urinalysis. Provider was in the bathroom, bathroom had stalls. Provider didn’t check my pockets are have me wash my hands b4 I used the bathroom. I urinated in cup. Handed cup to her. She checked temp then poured in 2 valves, I initialed both valves, and signed chain of command. Provider then said we now do a 2nd urinalysis that is supervised. I asked, when has this ever been the procedure, I’ve been taking these tests for 7yrs and this a first. She stated that this is there company policy. She said you can use the bathroom later but I must search you first. I was going to do it, until I thought that something wasn’t right about this. I began to challenge her on the legitimacy of this procedure. She began recording me, then another employee from Dart entered the bathroom. Dart employee instructed provider to go to another bathroom to administer drug test leave me where I was say I refused test. I didn’t refuse a test, plus I had already done one. Guy, who was in charge of orientation Quentin, tried to get provider to administer 2nd test, provider declined said she had reported on my record that I refused test. I waited there for 9hrs to be given test. I never received

I am only asking that a refusal for taking a drug test is not reported on my record.
